2019 National University System Web Survey Sweepstakes Official Rules

No purchase of any kind is necessary to enter or win. Making a purchase will not increase your chances of winning. Void where prohibited by law.

Entry into this Sweepstakes constitutes your acceptance of these Official Rules.

DATES OF SWEEPSTAKES: The “2019 National University System Web Survey Sweepstakes” (the “Sweepstakes”) shall begin at 12:00 a.m. (“ET”) June 19, 2019 and ends at 11:59 p.m. ET on July 31, 2019 (the “Sweepstakes Period”).

ELIGIBILITY: This Sweepstakes is open only to permanent legal U.S. residents of one of the fifty (50) United States or the District of Columbia (excluding residents of New York, Rhode Island, Florida, New Jersey and Puerto Rico and any other U.S. possession or territories), at least eighteen (18) years of age or older at time of entry (“Entrant”). The Entrants who are selected as the potential prize winners (each a “Prize Winner” and collectively, “Prize Winners”) must each have a unique, personal and valid social security number for Sponsor (defined below) to report the tax liability associated with acceptance of the respective prize (“Prize”). Valid social security numbers will be determined by Internal Revenue Service (“IRS”) requirements as further defined in the “Prize Claims” section of these Official Rules. Officers, directors, managers and employees of Q2 Insights, Inc. (“Sponsor”), of National University System providing marketing and market research services to Sponsor or its client(s), consultants and agents, as well as the immediate family (spouse, parents, siblings and children) and household members of each such person, and any others engaged in the development, production, execution or distribution of this Sweepstakes are not eligible to participate. Sweepstakes is void in New York, Rhode Island, Florida, New Jersey and Puerto Rico, any other U.S. possession or territories, and wherever else restricted or prohibited by law.

HOW TO PARTICIPATE: For this Sweepstakes two (2) Prize Winners will be selected from the eligible entries received. See “Random Drawing/Notification” section for complete details.

HOW TO ENTER: Online entry via the survey: You may receive an email request to complete a customer survey (the “survey”) during the Sweepstakes Period. The email will contain a link that will direct your browser to a McLaughlin & Associates Web Survey page. Upon completion of the survey you will be prompted to submit your email to the Sweepstakes.

RANDOM DRAWING/NOTIFICATION: On or about first day after the end of the Sweepstakes Period (July 31, 2019), at the Sponsor’s office located for purposes of the Sweepstakes at 566 South Route 303, Blauvelt, NY 10913, there will be two (2) Prize Winners chosen for the Sweepstakes Period and each will receive a prize as outlined in the “Prizes” section below. The random drawing will be conducted by a Sponsor representative, whose decisions are based upon their sole discretion and interpretation of these Official Rules, and are final and binding in all respects.

The potential Prize Winners will be notified by the Sponsor by email and/or telephone and each must complete the “Required Documents” as defined in the “PRIZE CLAIMS” section below (“Notification”). In the event the Sponsor cannot reach a potential Prize Winner via the Notification, if a Potential Prize Winner fails to respond to the Notification by the date indicated, or if it is determined by Sponsor, in its sole discretion, that a potential Prize Winner is not eligible in accordance with these official rules, an alternate Prize Winner will be selected at random from among the remaining non- winning eligible entries received during the Sweepstakes Period. In such case, he/she will be notified as outlined above.

ODDS OF WINNING: The odds of being selected as the Prize Winner will depend upon the total number of eligible entries received during the Sweepstakes Period.

PRIZES: Each of the two (2) Prize Winners will receive a $500 Visa gift card.

Each Prize is subject to the terms of the conditions of the gift card itself. Sponsor will not replace any lost or stolen Prizes. Each Prize is taxable as income and each Prize Winner is solely responsible for all applicable federal, state and local taxes, including taxes imposed on his/her income. No more than the stated number of Prizes set forth in these official Sweepstakes rules will be awarded. All Prizes will be awarded.

GENERAL CONDITIONS: By participating, Entrants agree to abide by and be bound by these Official Rules and the decisions of the Sponsor, which shall be final and binding with respect to all issues relating to this Sweepstakes. Should any portion of the Sweepstakes be, in Sponsor’s sole discretion, compromised by non- authorized human intervention or other causes which, in the sole discretion of the Sponsor, corrupt or impair the administration, security, fairness or proper submission of entries, Sponsor reserves the right, at its sole discretion, to cancel, terminate, modify or suspend the Sweepstakes and conduct the Sweepstakes in a manner which in its sole discretion is fair, appropriate and consistent with these Official Rules. Notice of such action by Sponsor will be posted at the Website. In such event, Sponsor reserves the right to award the prize(s) at random from among the eligible entries received up to the time of the impairment. Sponsor reserves the right in their sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Sweepstakes or to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner. Sponsor reserves the right to disqualify or reject any incomplete, inaccurate or false entry. Any attempt by any person to deliberately undermine the legitimate operation of the Sweepstakes is a violation of criminal and civil law, and, should such an attempt be made, sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law. Sponsor’s failure to enforce any term of these official rules shall not constitute a waiver of these provisions.

RELEASE: By participating in this Sweepstakes, Entrants agree to release and hold harmless Sponsor, National University System and its client(s), and each of their respective parent companies, subsidiaries and affiliates and the respective officers, directors, managers, employees, subsidiaries, affiliated entities, agents, members, shareholders, insurers, servants, guests, divisions, predecessors, successors, representatives, assigns, attorneys, dealers, distributors, advertising, promotion, and fulfillment agencies of each of these entities, including the Independent Administrator, (collectively, the “Released Parties”) from and against any claim or cause of action arising out of participation in the Sweepstakes or receipt, redemption or use of any Prize.

LIMITATIONS OF LIABILITY: The Released Parties do not have any responsibility or liability whatsoever for: (a); unauthorized human intervention in any part of the entry process or the Sweepstakes; (b) electronic or human error which may occur in the administration of the Sweepstakes or the processing of entries; (c) any property loss, damage, personal injury, or death, in connection with the prizes provided pursuant to this Sweepstakes and/or any person’s participating in the Sweepstakes; (d) any printing or typographical errors in any Sweepstakes-related materials; (e) stolen, lost, late, misdirected, corrupted, incomplete, damaged or indecipherable entries; or (f) technical failures of any kind, including but not limited to the malfunctioning of any computer, mobile device, cable, network, hardware or software (if applicable), as well as any failure of the Website during the Sweepstakes Period.

CHOICE OF LAW: Under no circumstances will Entrants be permitted to obtain awards for, and Entrants hereby waive all rights to claim, punitive, incidental, or consequential damages, or any other damages, including attorneys’ fees, and Entrants further waive all rights to have damages multiplied or increased. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of Entrants and Sponsor in connection with this Sweepstakes, shall be governed by, and construed in accordance with, the substantive laws of the state in which National University System is located, without regard to such state’s conflict of law principles.

ENTRANTS PERSONAL INFORMATION: When you participate in the Survey and enter this Sweepstakes, Sponsor will be collecting non-personally identifiable data about Entrants. Sponsor will also collect your name, mailing address, and phone number. We use this information to administer your participation in the Sweepstakes. All other information collected from Entrants (including but not limited to the Entrant’s gender and any other demographic information) is collected for analysis purposes only.
